Reporting a Loss

I got into FOREX late last year and ended with a loss so section 988 is the way to go for me. What IRS tax form do I need to fill out?

There would be no form other than your 1040.

I do not provide instructions for tax forms so I would recommend checking with your tax preparer or looking at the publication.
